HOME CARE PATIENT TRACKING SYSTEM
The Broome County Health Department is committed to preserving, promoting, and protecting the public health and well-being of Broome County residents by preventing diseases, protecting the environment and promoting healthful living. In Broome County Health Department, services are available to help older individuals remain independent in their homes when poor health and/or difficulty with activities of everyday life make it difficult. So in our project; a home care patient tracking system is wanted from us to be made. Broome County Health Department works with agencies. They are like Lourdes At Home, Interim, Professional Home Care etc. Each agency takes care of the related patients in their nearest area. In emergency conditions such as: flood, snow, electricity outage etc.; Broome County Health department warns the agencies about the situation. Then each agency procures all the needs of the patients in their area. But to do these in a more organized way; Broome County Health Department needs a system to track and control the existing information on a single computer based program. Hence they will be able to provide a better service at the right place on time.
In the first web page of computer based program, login form is seen. A username and password is needed to log on. There are also “Documents”, “Demo”, “F.A.Q” and “Contact” links on the left side of the page.
There are two types of users; agency or admin users.
If the username belongs to an agency, then the agency dashboard of that user is displayed. At first look in the dashboard; you see the “on call personnel” of that agency. Then on the left side of the page, you see “License Check”, “NYS Phone Numbers”, “Contact Broome County”, and “HCC EP Workgroup” links. In the agency dashboard, there are two main functions: manage patients and manage staff. When manage patient is selected, the user will be able to add patient, remove patient, make inactive and active of a patient, view details of a patient, and edit patient. When manage staff is selected, the user will be able to add staff, remove staff, make inactive and active of a staff, make a staff on call, view details of a staff and edit staff. In addition to these; in the dashboard three priority levels, electricity outage and oxygen-needed boxes are seen. In the boxes you can see the patient numbers those belong to these priority levels, electricity outage and oxygen-needed boxes.
If one of the priority levels is selected, a patient list is displayed according to that priority level. For example; if priority level 2 box is selected than the patients that are under priority level 2 is shown. After you get the patient list, you can see all the information about these patients.
If the username belongs to an admin, then Broome County dashboard of that user is displayed. At first look in the dashboard; you see the link “Click here full list”. When you click this, you will get the list of agencies. The agency table will contain the fields; name, primary contact, phone number, number of staffs, number of patients and map. Map is a link, so if you click it, it will show the address of the primary contact, which belongs to the agency which you selected, from the google map. On the left side of the dashboard, you see “Electricity Outage”, “Oxygen-Needed”, “Agency County List”, “Staffing / Municipality”, “Municipality / Priority”, “Staff Contact List”, “Comprehensive Report”, “Export to GIS” links. In addition to these; there are 3 sections that will be displayed on Broome County dashboard. In section 1; the priority levels, electricity outage and oxygen-needed boxes will be seen. If one of them is selected, a patient list is shown according to the selected property. In the table of the patient list, there will be name, phone, address, municipality, city, details, and GIS fields. In section 2; Municipality/ HC Patient Cross table is seen. Municipalities contain different number of patients according to their priority levels. If a municipality is selected, then all the patients in that municipality are seen. If a priority is selected in a municipality, then the patients that belong to that municipality and priority level will be seen. In section 3; On-Call personnel of agencies are seen in a table. In the table, there will be agency name, primary contact and contact phone fields.
If the electricity outage is selected from the left menu of Broome County dashboard; Electricity-Outage form is displayed. Then you should select the municipalities and press the submit button. When it is submitted; the agencies are informed and their electricity charts change.
If the oxygen-needed is selected from the left menu of Broome County dashboard; Oxygen-Needed form is displayed. Like the Electricity-outage table; you should chose some municipalities and press submit button. But this time, the agencies are informed and their oxygen charts change.
